Intake and Enrollment

Who We Serve

FUCIS provides day program services to adults with intellectual and developmental disabilities who are eligible for Regional Center services under Service Code 531 (Community-Based Day Program), Tailored Day Services (TDS) and Self-Determination (SDP) participants.

To be eligible for enrollment, individuals must:

    • Be 18 years of age or older
    • Is an active Regional Center eligibility consumer
    • Be ambulatory or able to participate in community activities using a mobility aid (cane, walker, or self-propelled wheelchair) with minimal assistance
    • Be able to communicate basic needs through verbal language, sign, a communication device, or another reliable system
    • Be able to self-administer medications during program hours, if medications are needed
    • Be medically stable, with no condition requiring continuous skilled nursing supervision
    • Be able to participate safely in integrated community environments, responding to verbal staff support without requiring physical intervention
    • Benefit from services offered, as reflected in their Individual Program Plan (IPP) goals and long-term objectives

Our Intake Process

  • Step 1: Initial Assessment Meeting Once we receive a referral, FUCIS schedules an assessment meeting with the individual and their family at a location of their choice — a local café, their home, our FUCIS office, or by Zoom. This is a relaxed, no-pressure conversation where the individual and family can ask questions about FUCIS services, and our team can learn more about the person we may have the privilege of supporting.

    Step 2: Confirming the Match If the individual and family feel FUCIS is the right fit — and we feel confident we can fully support the individual’s goals — we ask the family to reach out to their Regional Center Service Coordinator and share their interest in beginning services with FUCIS. The Service Coordinator will then contact our office to confirm services and initiate the enrollment paperwork process.

    Step 3: Intake Packet & Medical Clearance FUCIS will provide the individual and family with our Onboarding Intake Packet to complete and return. A current TB test result and physical exam clearance are also required as part of enrollment. Once the Regional Center has authorized services and all paperwork has been received, the individual is ready to begin their FUCIS journey.

    Step 4: Two-Day Onboarding Orientation Before the first official day of services, every new participant is invited to our two-day onboarding orientation — designed to set them up for a confident, successful start. (See the section below for more details.)

Two-Day Onboarding Orientation

What makes FUCIS different begins before services even start.

Every new participant is invited to a two-day onboarding orientation prior to their first day of services — facilitated by the same Trainer who trains every FUCIS staff member. This shared starting point means participants and their support team begin services with the same language, the same expectations, and the same goals in mind.

During orientation, participants gain:

  • A clear understanding of their individual rights and HCBS protections
  • An introduction to person-centered planning and how their voice shapes every decision
  • A walk-through of what a typical day at FUCIS looks like
  • An overview of PIP, volunteerism, vocational training, and remote learning pathways
  • Travel training for Access Paratransit, public bus, and Metro rail
  • Hands-on practice designing their own daily schedule
  • A “getting to know you” meet-and-greet with their potential support staff to confirm a good match before services begin

We believe this two-day investment is one of the most important steps in setting every participant up for a confident, lasting experience at FUCIS — and we partner closely with families and Service Coordinators to make sure it happens.
